1. The date listed below is what we will strive to have your orders ready by. However this date IS NOT FIRM and is subject to change as it is pending the approval of all personalization.

2. All personalization must be approved by the WVU Hospital Licensing and Marketing Department. We will submit all personalization to them for approval. IF SOMETHING IS NOT APPROVED IN YOUR PERSONALIZATION, IT WILL BE REMOVED.

3. If you opt for personalization please enter the information like (example): John Doe, MD / EMERGENCY MEDICINE. Please SPELL out your department (do not abbreviate).

4. Do not list your floor as the department. It will not be approved by WVU Hospital Marketing.

5. Please make sure the correct size is ordered and that your personalization is entered correctly. You must add in your department if you want it. We do not assume/add it in automatically. Underground Printing will not refund or replace items ordered incorrectly. If you do not add personalization, your order will be printed with the logo on the left chest only.

6. If you are adding personalization, please choose from the following options for the department:
