WVU Extension Service Apparel TAX EXEMPT
**PLEASE READ CAREFULLY BEFORE ORDERING**1. Order off this form if you need to use your WVU P Card to pay and need the tax exempt status
2. You will be emailed an invoice to pay for your order after it is placed on this order form. Shipping fees will be included on the invoice in a section labeled 'other'. Shipping is $6.95 for the first item and an additional $1.00 for every item ordered after. If you order three items your shipping fee would be $8.95, etc.
3. The cost of each item is listed in the main item description
4. Invoices must be paid before the orders ship out. Not doing so may result in the cancellation of your order.
